Medical Social Worker – Two Positions

Social Worker

The social worker develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.

A Master’s in Social Work, Counseling or related field is preferred

Position Available: Part-time, 24 hours per week + PRN, Days, Varying Shifts, Rotating Weekends and Call Coverage Required.

Responsibilities:

Assesses the functional and psychological needs of the patient within the framework of his/her developmental stage, functional abilities, cultural milieu, and support network.

Develops a plan of intervention in collaboration with the healthcare team; facilitates adjustment in the care plan or services when necessary to promote enhanced outcomes.

Documents all interventions in the patient medical record both timely and accurately including all elements of the discharge plan.

Provides advocacy, assistance and support to patients and families. In the case of suspected abuse/neglect/exploitation, the Social Worker documents and reports where required. Acts as a liaison to hospital and community groups.

Functions in a liaison role between the hospital and community in making community resources available to the patient and family.

Collaborates with all members of the healthcare team to develop, manage and communicate discharge plans. Maintains current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term care and low-income patients.

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Qualifications:

Requires a Bachelor’s of Social Work or related degree.

Behavioral Health Licensure required as mandated by Alaska state board.

Requires a proficiency level typically achieved with 3 years acute care hospital experience. Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Social Worker in the specialty area.

Must have knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, human relations skills and time management skills. In acute care, must be able to work flexible hours and take rotating call after hours.

PREFERRED QUALIFICATIONS

Basic Life Support (BLS) certification preferred.

Additional related education and/or experience preferred.

Fairbanks Memorial Hospital Social Worker

Clinical Director

Job Title: Clinical Director

In accordance with Assets, Inc. core values and mission, the Clinical Director provides clinical oversight for all mental health services.  In collaboration with the Community Services Program Director, provides consultation and direction in developing and maintaining living and working environments in line with Assets mission.

Minimum Qualifications:

  • Master’s degree in psychology, counseling, social work or psychiatric nursing required; and four or more years related experience and/or training;
  • Mental Health professional clinician licensed to practice in Alaska; and
  • Three years providing support to individuals with psychiatric and/or developmental disabilities.

Must have an insured vehicle and current Alaska driver’s license (with no or low points).  Must be able to pass a criminal background check under state regulations. Must provide a copy of the driving record and proof of vehicle insurance and educational credentials and other professional trainings, if applicable.

Assets, Inc., a non-profit organization that strives to make a difference in the lives of people with developmental disabilities and/or mental illnesses is recruiting for a Full-Time MENTAL HEALTH CLINICIAN II.

The Mental Health Clinician II is responsible for providing clinical consultation, guidance, quality assurance, training and therapeutic services to individuals experiencing developmental disabilities and/or mental illnesses. This position provides consultation to direct support professionals, case managers and other treatment team members.

Essential Responsibilities: Adheres to and demonstrates Assets, Inc. core values, mission, policies, and compliance plan in the performance of all aspects of this position. Facilitates Mental Health Medicaid for individuals on clinician’s case load by: Overseeing and being involved in treatment plan development; Approving Mental Health Medicaid (MHM) treatment plans; Overseeing production of and approving Prior Authorization requests; Conducting and writing Comprehensive Assessments; Reviewing and approving Functional Assessments; As part of clinical team, preparing clinical portion of DBH grant, quarterly reports and assisting in other grants sought by Assets Inc. Represents Assets by collaborating with other providers to serve individuals whose diagnosis and treatment is at issue, following-up on external Assessments and recommendations to incorporate into Psychiatric Treatment as well as service design. In collaboration with the CS Department Director, assesses a potential client’s treatment and service needs and Assets’ capacity to meet them. Provides clinical consultation to treatment teams. Performs individual, group and family therapy.

Minimum Qualifications: Master’s degree in psychology, counseling, social work or psychiatric nursing required and three (3) to five (5) years of related experience. Alaska Licensed Professional Counselor or Psychological Associate or ability to become licensed within one year of employment. Must have Alaska driver’s license and insured vehicle or reliable transportation. Must be able to pass a criminal background check under the state regulations. Proof of driver’s license, insurance and educational credentials must be presented at time of hire.
